In-home care in Boston, MA: how to compare agencies

We ranked the top nine in-home care agencies serving Boston, MA by their public reviews. They are ordered by the Care Close to Home Score, which combines the review rating (adjusted for how many reviews stand behind it), the number of reviews and whether a phone number is listed. Each has at least 10 public reviews.

We have not verified state licenses for these agencies: Massachusetts does not publish a usable roster of non-medical home care agencies, so this list is based on reviews, not on a license check. Ask each agency for its license or registration number if the state requires one.

Few agencies are based in Boston, MA itself, so most of the agencies listed are based in nearby towns and serve Boston, MA. Ask any agency to confirm it covers your address.

In-home care means non-medical help that lets an older adult keep living at home: bathing and dressing, meals, housekeeping, errands, medication reminders and company. It is usually paid for privately, with long-term care insurance, a Medicaid program or VA benefits, not by Medicare. Compare at least three agencies, and ask each whether caregivers are employees, how they are screened, who supervises them, the minimum hours and the hourly rate.

In Massachusetts, the median rate for in-home care is $40 an hour, about $3,467 a month for 20 hours a week (CareScout 2025).

Questions Boston families ask

How do I choose an in-home care agency in Boston?

Call at least three. Ask whether caregivers are employees or independent contractors, how they are screened and supervised, the minimum hours and the hourly rate, and what happens when a caregiver is sick. Ask for a written care plan. Our guide to how to check an agency has the full list.

How are these agencies ranked?

The Care Close to Home Score is out of 100. About 70 points come from the review rating, adjusted so a handful of reviews counts for less than a hundred, about 24 from the number of reviews, and about 6 from having a phone number listed. Agencies cannot pay to rank higher, and the score does not include a state license check.

Are the agencies on this Boston list licensed?

We have not verified licenses for these agencies, because Massachusetts does not publish a usable roster of non-medical home care agencies. Ask each agency for its license or registration number if the state requires one, and confirm it with the state.

How do families pay for in-home care in Boston?

Most pay privately, by the hour. Long-term care insurance, Medicaid waivers and VA benefits can help with bathing help, meals, housekeeping or companionship.
